Rogues Gallery of Dangerous Conditions Pictured below are samples of items of equipment we have "failed" during our Inspection and Testing Schedules. Each case highlights the risks associated with poorly maintained equipment and the subsequent potential danger of electrocution, fire, and equipment failure. Thus ensuring the need for thorough visual inspections as well as electrical testing. 1. Sequence tells a Story ! Signs of overheating on both plugtop and socket.
The plugtop cover was removed to reveal extensive heat damage.

Side view of burn damage. Incredibly the appliance in question was still working under these conditions. Plug removed to ensure no further use.
2. Outer Insulation Damage

3. Mains Adaptor Misuse
This mains adaptor was being used in an inappropriate environment, where solvents and their fumes were present. Explosion and fire risk evident.
4. Non compliant Extension Lead
Internal view of "extension lead" constructed using 2 core cable! As can be seen there is no earth wire. All extension leads / trailing sockets should be constructed with 3 core cable providing earth protection and correct polarity and protected with a 13 amp fuse. 5. Earth Disconnected
This welding appliance had lost its casing earth terminal and, as can be seen, the earth lead is adrift. In the event of an electrical fail condition the casing would become live, exposing operators to electrocution. |
